The Questions are provided to us by Welsh Assembly Government and include a section on equality monitoring. If you prefer you can complete this form electronically, please visit the Practice Website or Facebook page for a link or use above QR Code. Your NHS Wales Experience The experience that you have of care is important to us. This might be an appointment with your doctor or health visitor, a hospital stays, an outpatient visit or something else. We would be grateful if you could complete this survey so that we can understand this better.

The questions are based on the things that patients have said matter most. We will ask you questions about your latest experience of healthcare. Please help us by giving your honest opinion.

The questions mostly have 4 options, and you are asked to tick the answer that you feel best describes how you feel.   Some of the questions have ‘not applicable ‘. Please tick this if the question is not relevant to your experience.
 We do not need to know your personal details but have asked some general questions at the end about who you are. This is so we can make sure we are asking all groups of people about their experience.

The First Contact Physiotherapist (FCP)

First Contact Physiotherapists (FCP) have extensive experience in the assessment and management of musculoskeletal conditions – such as back or neck pain – and see these patients without the need for a GP appointment.

What to expect when you see your FCP

  • Your FCP will assess and diagnose you.
  • Give you expert advice on how to best manage your condition
  • Refer you onto a specialist service if necessary.

How do you book an appointment to see your FCP?

  • Simply contact your GP Practice and request an appointment to see the FCP. The reception team will check that you’re suitable and book an appointment for you.

 

New way to access Mental Health & Wellbeing Support

New way to access Mental Health & Wellbeing Support

Hywel Dda University Health Board is the first health board in Wales to launch a dedicated mental health advice service, offering support for all age groups through the established 111 call line.

People will be able to access the service by phoning 111 and selecting option 2 where they will be put through to a mental health practitioner.

The service is accessible 7 days a week 24 hours a day

Beat Flu this Winter

  • BOOK YOUR FLU VACCINE NOW!!!

Flu is more likely to be serious if you have a long term health condition, are pregnant, or are older.  The people at high risk of COVID-19 are generally the same people at increased risk of becoming very ill with flu.

Flu can also be serious for young children.

Lat year in Wales more than a million people got their flu vaccines, that’s around one in every three people.

If any of the following apply to you , even if you feel healthy, you are more likely to get complications from flu if you catch it and you are advised to have a flu & COVID vaccine.

  • You are Pregnant
  • You are aged 65 or over (AGE 65 years by 31 March 2026, born before 1 April 1961)
  • you are aged six months to 64 years old in a clinical risk group including (COPD, Chronic Heart Disease, CKD stage 3, 4 or 5, Chronic Liver Disease, Chronic Neurological Disease such as Parkinson’s or Motor Neuron Disease, Diabetes, Epilepsy, Immunosuppression, BMI over 40 
  • You live in a care home 
The following groups are also advised to have a flu  vaccine to protect them and the people around them. 
  •   Children aged two or three years old (age on the 31st August 2025) i.e date of birth on or after 1 September 2021 and on or before 31 August 2023.
  • Children in Primary school reception class and school years 1 – 6
  •  Children and young people in school years 7 to year 11
  • Carers
  • People with learning disabilities
  • People working directly with patients/clients in health or social care
  • Healthcare workers (including healthcare students) with direct patient contact
