As part of the General Medical Services Contract, Practices are required to ask a cross-section of patients about their experiences of using NHS Wales services.  The Questions are provided to us by Welsh Assembly Government and include a section on equality monitoring.

Alternatively you can ask at reception for a paper copy to complete. 

The survey will run from 1st July 2025 to 31st January 2026.  Results of the survey will be made available on this page in spring 2026.
